Una vez completado el proceso de dilatación cervical y el descenso de la presentación, la parturienta será trasladada a la sala de partos. La asistencia comienza con la dilatación completa.
Objetivos de enfermería en esta etapa:
ü Estimular a la parturienta durante los pujos para que se relaje y ventile adecuadamente entre las contracciones.
ü Favorecer la relajación entre las contracciones
ü Promover la participación activa de la gestante y de un miembro del grupo familiar que la acompañe durante la expulsión del feto.
ü Establecer el contacto precoz madre e hijo y favorecer la lactancia.
ACCIONES
v Colocar a la parturienta en la camilla o sillón de parto, en una posición cómoda. Se recomienda la posición sentada o semi-sentada, ya que fisiológicamente es la que permite que la parturienta realice los pujos con eficacia favoreciendo el descenso y la expulsión del feto. Desde el punto de vista anatómico, permite que la expansión torácica y el descenso del diafragma sobre los órganos abdominales. Desde esta posición también se puede ver en nacimiento del bebé

v Control de los latidos cardiacos fetales. Se considera normal el descenso de la frecuencia cardiaca hasta 100 latidos por minuto por compresión, durante la expulsión del polo cefálico.

v Ayudar y guiar los pujos según la dinámica uterina. Durante las contracciones los pujos deben ser originados por la parte del diafragma sobre los órganos abdominales. Si la parturienta realiza la fuerza a nivel de la cara y cuello, no favorece el descenso del feto atreves del canal de parto y conduce al agotamiento de la mujer.
El pujo materno espontaneo suele ser suficiente para la expulsión fetal lo que se constata por el progreso, descenso y la rotación de la presentación.
El mecanismo del parto tiene cuatro tiempos : En cada etapa el feto cambia de actitud y realiza movimientos de rotación para acomodarse a los distintos diámetros y ejes de la pelvis. Esta acomodación es resultante de las fuerzas de la contracción y de los obstáculos opuestos por el canal de parto.
Ø Descenso: desplazamiento sostenido del feto hacia abajo, producido por la presión de las contracciones uterinas
Ø Flexión: actitud natural que se ajusta a la forma de la cavidad uterina Y LA resistencia del suelo pélvico durante el trabajo de parto.
Ø Rotación interna: cuando la cabeza fetal llega a la parte media de la pelvis, debe girar hacia posición occipital para adaptarse al diámetro transverso.
Ø Extensión: la presión sostenida de las contracciones uterinas hacia abajo y la resistencia del suelo pélvico, producen extensión de la cabeza fetal, esta se mantiene extendida hasta el coronamiento mientras se distiende el perineo.
Ø Restitución: movimiento automático que ocurre cuando la cabeza sale por el perineo, y consiste en un ajuste del cuello del feto hasta que la cabeza guarde relación normal con los hombros.
Ø Rotación externa y expulsión: esta etapa puede ocurrir junto con la restitución. Consiste en rotación de los hombros del feto y la salida del hombro anterior por debajo de la sínfisis del pubis, a la vez el hombro posterior se desliza por el perineo.

 Aims of infirmary in this stage:

ACTIONS
v To place the parturient in the stretcher or armchair of childbirth, in a comfortable position. There is recommended the seated or semi-seated position, since physiologically it is the one that allows that the parturient should fulfil the pujos with efficiency favoring the decrease and the expulsion of the fetus. From the anatomical point of view, it is allowed that the thoracic expansion and the decrease of the diaphragm on the abdominal organs. From this position also it is possible to see in birth of the baby.
v Control of the cardiac foetal beatings. Is considered to be normal the decrease of the cardiac frequency up to 100 beatings for minute by compression, during the expulsion of the cephalic pole.
v To help and to guide the pujos according to the uterine dynamics. During the contractions the pujos must be originated on the part of the diaphragm on the abdominal organs. If the parturient realizes the force to level of the face and neck, it does not favor the decrease of the fetus you dare of the channel of childbirth and he drives to the depletion of the woman.
 I bid mother spontaneously it is in the habit of being sufficient for the foetal expulsion what is stated by the progress, decrease and the rotation of the presentation.
The mechanism of the childbirth has four times: In every stage the fetus changes attitude and realizes movements of rotation to get accommodated to the different diameters and axes of pelvis. This accommodation is resultant of the forces of the contraction and of the obstacles objected by the channel of childbirth.
Ø Decrease: displacement supported of the fetus down, produced by the pressure of the uterine contractions
Ø Flexion: natural attitude that adjusts to the form of the uterine cavity AND THE resistance of the pelvic soil during the labor.
Ø Rotation hospitalizes: when the foetal head comes to the average part of the pelvis, it must turn towards occipital position to adapt to the transverse diameter.
Ø Extension: the pressure supported of the uterine contractions down and the resistance of the pelvic soil, they produce extension of the foetal head, this she is kept extended up to the crowning while the perineum is distended.
Ø Restitution: automatic movement that happens when the head goes out for the perineum, and it consists of an adjustment of the neck of the fetus until the head guards normal relation with the shoulders.
Ø External rotation and expulsion: this stage can happen together with the restitution. It consists of rotation of the shoulders of the fetus and the exit of the previous shoulder below the sínfisis of the pubis, simultaneously the later shoulder slips for the perineum.
